28501! Job Title: Case Manager - RN Profession: Registered Nurse

required. License: Licensed to practice as a Registered Nurse in
the state. Certifications: BLS required. ACM/CCM preferred.

required. - Must reside outside a 50-mile radius. Description: The
purpose of this position is to provide ongoing support and
