HOME CARE LPN OFFICE NURSE Liberty Cares With Compassion At

seeking an experienced: HOME CARE LPN OFFICE NURSE Full Time (New
Hanover/Pender/Duplin Counties) Job Description: - Provides direct
patient care, to include using a variety of skills and necessary
coordination of services. - Completes appropriate documentation as
required by policy and procedure and regulation. - Assists Patient
Care Coordinator and/or Operations Manager in day to day branch
office tasks to ensure patient care documentation is complete and
submitted timely. - Assists Patient Care Coordinator and/or
Operations Manager in management of patient care supplies. -
Manages all commercial insurance payers and Medicare replacement
plans for continued authorization regarding home care visits and
documents authorization in electronic medical record per software
system. - Coordinates with Operations Manager and Patient Care
Coordinator in the management of Face to Face documentation as
required by specific payer source. Job Requirements: - Current LPN
licensure within the state of branch location, and successful
completion of nursing education from an approved school of nursing.
- One year of clinical nursing experience, preferably in an acute
care medical/surgical setting. - One year of Home Care experience
preferred. - Ability to independently organize work and provide
effective health care services to homebound patients. - Effective
communication skills (oral and written). - CPR certified. Visit
