A-Line Staffing is now hiring LTSS Service Care Manager (RN or LCSW) in Fayetteville, NC and surrounding areas . The LTSS Service Care Manager would be working for a Major Health Plan and has career growth potential. LTSS Service Care Manager Highlights: Schedule: Monday–Friday, 8:00 AM – 5:00 PM EST (Remote with 80% local travel) Pay Range: $40-45 Hourly, plus mileage reimbursement LTSS Service Care Manager Responsibilities: Manage a caseload of healthcare members with long-term care needs Conduct monthly and quarterly assessments via telephonic and in-person visits Complete evaluations with members, caregivers, and providers to inform care planning Monitor service delivery and ensure appropriate referrals and authorizations Coordinate care plans with providers, discharge planners, and other parties Assist members with appeals and complaints as needed Ensure services are delivered without gaps and plans of care are optimized LTSS Service Care Manager Qualifications: Required : Active Registered Nurse (RN) or Licensed Clinical Social Worker (LCSW) license in NC Valid Driver’s License (must reside within a 2.5–3 hour radius of service area) 2 years of care management experience Background in physical health, utilization management, and/or home health Preferred : 4–6 years of care management experience Experience with long-term care, discharge planning Familiarity with EMR systems and Microsoft Office If you are interested in this LTSS Service Care Manager position, please apply to this posting with Luke Heuser at A-Line!
